Background Encounters of moral distress have long-term consequences on healthcare workers’ physical and mental health, leading to job dissatisfaction, reduced patient care, and high levels of burnout, exhaustion, and intentions to quit. Yet, research on approaches to ameliorate moral distress across the health workforce is limited. Research Objective The aim of our study was to qualitatively explore multi-professional perspectives of healthcare social workers, chaplains, and patient liaisons on ways to reduce moral distress and heighten well-being at a southern U.S. academic (...) medical center. Participants & Research Context Purposive sampling and chain-referral methods assisted with recruitment through hospital listservs, staff meetings, and newsletters. Interested participants contacted the principal investigator and all interviews were conducted in-person. Consent was attained prior to interviews. All interviews were recorded and transcribed verbatim. Research Design Directed content analysis was used to deductively organize codes and to develop themes in conjunction with the National Academy of Medicine’s National Plan for Health Workforce Well-Being. Rigor was attained through peer-debriefing, data triangulation methods, and frequent research team meetings. Ethical Considerations Ethics approval was obtained from the university and medical center institutional review boards. Findings Themes demonstrate that rather than offering interventions in the aftermath of moral distress, multilevel daily practices ought to be considered that pre-emptively identify and reduce morally distressing encounters through (1) the care team, (2) management and leadership, and (3) the health care industry. Strategies include interdisciplinary decision-making, trusting managerial relationships, and organizational policies and practices that explicitly invest in mental health promotion and diverse leadership opportunities. Conclusion Moral distress interventions ought to target short-term stress reactions while also addressing the long-term impacts of moral residue. Health systems must financially commit to an ethical workplace culture that explicitly values mental health and well-being. (shrink)

Religious individuals often assume that their beliefs promote strong romantic relationships. Yet the empirical evidence is mixed. To better understand this association, this study examined religious orientation as a moderator within the investment model of commitment. A community sample of 84 couples completed measures on religious orientation and commitment as part of a larger study on romantic relationships. The findings indicate that although both religious motivations promote commitment, they do so differently. Specifically, intrinsic religious orientation buffered the negative effects of (...) dissatisfaction in a relationship; whereas an extrinsic religious orientation attenuated the extent to which alternatives to the relationship influenced commitment. Future research in this area should consider religious orientation, rather than overall religiosity, and examine specific predictors of relationship commitment to provide greater clarity on how religiosity influences romantic relationships. Implications for counseling individuals with religious beliefs are also discussed. (shrink)

Researchers from diverse disciplines, including organismal and cellular physiology, sports science, human nutrition, evolution and ecology, have sought to understand the causes and consequences of the surprising variation in metabolic rate found among and within individual animals of the same species. Research in this area has been hampered by differences in approach, terminology and methodology, and the context in which measurements are made. Recent advances provide important opportunities to identify and address the key questions in the field. By bringing together (...) researchers from different areas of biology and biomedicine, we describe and evaluate these developments and the insights they could yield, highlighting the need for more standardisation across disciplines. We conclude with a list of important questions that can now be addressed by developing a common conceptual and methodological toolkit for studies on metabolic variation in animals. (shrink)

The thymus is a unique primary lymphoid organ that supports the production of self‐tolerant T‐cells essential for adaptive immunity. Intrathymic microenvironments are microanatomically compartmentalised, forming defined cortical, and medullary regions each differentially supporting critical aspects of thymus‐dependent T‐cell maturation. Importantly, the specific functional properties of thymic cortical and medullary compartments are defined by highly specialised thymic epithelial cells (TEC). For example, in the medulla heterogenous medullary TEC (mTEC) contribute to the enforcement of central tolerance by supporting deletion of autoreactive T‐cell (...) clones, thereby counterbalancing the potential for random T‐cell receptor generation to contribute to autoimmune disease. Recent advances have further shed light on the pathways and mechanisms that control heterogeneous mTEC development and how differential mTEC functionality contributes to control self‐tolerant T‐cell development. Here we discuss recent findings in relation to mTEC development and highlight examples of how mTEC diversity contribute to thymus medulla function. (shrink)

Despite the salient experience of encoding threatening events, these memories are prone to distortions and often non-veridical from encoding to recall. Further, threat has been shown to preferentially disrupt the binding of event details and enhance goal-relevant information. While extensive work has characterised distinctive features of emotional memory, research has not fully explored the influence threat has on temporal memory, a process putatively supported by the binding of event details into a temporal context. Two primary competing hypotheses have been proposed; (...) that threat can impair or enhance temporal memory. We analysed two datasets to assess temporal memory for an in-person haunted house experience. In study 1, we examined the temporal structure of memory by characterising memory contiguity in free recall as a function of individual levels of heart rate as a proxy of threat. In study 2, we replicated marginal findings of threat-related increases in memory contiguity found in study 1. We extended these findings by showing threat-related increases in recency discriminations, an explicit test of temporal memory. Together, these findings demonstrate that threat enhances temporal memory regarding free recall structure and during explicit memory judgments. (shrink)

"In Learning from Our Mistakes: Epistemology for the Real World, Talbott provides a new framework for understanding the history of Western epistemology and uses that framework to propose a new way of understanding rational belief. His proposal makes epistemology relevant to the real world, which he illustrates with a new theory of racial, gender and other kinds of prejudice, a new diagnosis of the sources of the inequity in the U.S. criminal justice system, and insight into the proliferation of (...) tribal and fascist epistemologies based on alt-facts and alt-truth. Talbott's new model of rational belief is not a model of a theorem prover in mathematics. It is a model of a good learner. Being a good learner requires sensitivity to clues, the maginative ability to generate alternative explanatory narratives that fit the clues, and the ability to select the most coherent explanatory narrative. Sensitivity to clues requires sensitivity not only to evidence that supports one's own beliefs, but also to evidence that casts doubt on them. One of the most important characteristics of a good learner is the ability to correct mistakes. Talbott articulates nine principles that help to explain the difference between rational and irrational belief. Talbott contrasts his approach with the approach of historically important philosophers, including Socrates, Plato, Aristotle, Hume, Kant, Wittgenstein, and Kuhn. He also contrasts his approach with a variety of contemporary approaches, including pragmatism, Bayesianism, and naturalism. James Edward Meade was a renowned British economist who, alongside Bertil Ohlin, was joint winner of the 1977 Nobel Prize in Economics. Originally published in 1958, this book contains the transcription of Meade's inaugural lecture as Professor of Political Economy at Cambridge University, delivered during the same year. Concise and highly readable, it will be of value to anyone with an interest in economic history, theories regarding inflation, and macroeconomic theory in general.

In this article, we examine the ways gendered frames shift to make room for societal changes while maintaining existing pillars of systemic gender inequality. Utilizing the case of U.S. media representations of transgender people who reproduce, we analyze how media outlets make room for increasing societal recognition of transgender people and maintain cisnormative and repronormative traditions and beliefs in the process. Specifically, we outline how these media outlets accomplish both outcomes in two ways. First, they reinforce cisgender-based repronormativity via conceptualizations (...) of transgender reproduction as new and occurring in contrast to normative, cisgender reproduction. Second, they create a transnormative reproductive subject, which establishes a new socially sanctioned script for what it means to be transgender and what types of transgender experience may be recognized or accepted in mainstream society. In conclusion, we draw out implications for understanding how social authorities may shift existing gender frames to make room for changes in society while at the same time maintaining normative beliefs. These normative beliefs continue to facilitate societal patterns of gender inequality within such new frames. (shrink)
